Printing Player Forms

All coaches are required to maintain possession of all player registration and concussion awareness forms at all times while in the presence of the team (i.e. practice and games). The league, due to privacy restrictions, does not have access to these forms. A copy must be provided to the coach by each of the parents before any practices can commence.

To print these forms:

1. Go to ayso1455.org and click on Login in the top right
2. Once logged in, the default page is My Account and you will see a list of your participants in the main window
3. Find the player in question and click on View/Print Form
4. For a physical printout, select your printer and click Print -or- for an electronic copy choose the printer destination as Save to PDF
